Notice of Public Hearing Borough of Mountainside County of Union, State of New Jersey Please take notice that the Planning Board (the “Board”) of the Borough of Mountainside (the “Borough”) will, on July 28, 2015, at 7:30 p.m., at the first floor courtroom, Mountainside Borough Municipal Hall, 1385 U.S. Highway 22 East, Mountainside, NJ 07092, or at such other time and place as the Board may adjourn thereafter, hold a public hearing (the “Hearing”) to consider an application for preliminary and final major site plan approval and variance relief (the “Application”) made by CSH Mountainside, LLC (the “Applicant”) to permit an approximately 23,143 sq. ft., 3-story, 79-unit “assisted living” facility (the “Project”) at real property identified as Lots 7, 7A, 8A, & 8B of Block 24D on the Borough’s tax map (the “Property”). In addition to the foregoing, the Project will include typical, ancillary site improvements such as garbage disposal areas, internal driveways, lighting, landscaping and parking areas. The Property is commonly known as 1042-1050 Springfield Avenue, Mountainside, NJ. The Property consists of approximately 1.84 acres within the Borough’s O-B Office Building District Zone (the “O-B Zone”). To permit the Project, the Applicant will seek variance relief from the Borough’s Land Use Ordinance (the “Ordinance”) in accordance with the New Jersey Municipal Land Use Law, N.J.S.A. 40:55D-1, et. seq. as follows: (1) “d(1)” or “use” variance relief to permit an “assisted living” facility in the O-B Zone whereas same is not a permitted use; (2) “d(6)” or “c” variance relief, as determined by the Board to be necessary, to permit a 3-story building height that exceeds by 10 ft. or 10% of the height required in the O-B Zone, which, by Ordinance, is a maximum of 2-stories or 35-ft. in height, whichever is the lesser; and (3) to the extent determined by the Board to be necessary, “c” or “bulk” variance relief for (i) a front yard setback of 43.3 ft. whereas 50 ft. is required by Ordinance and (ii) a maximum ground projection of 30% whereas 25% is the maximum permitted by Ordinance. The Applicant will also seek any variances, waivers, exceptions, or other relief that the Board or its professionals may determine to be necessary without additional public notice in accordance with the MLUL. A copy of the Application and supporting documentation is on file with the Board’s Secretary. These documents are available for public inspection, during regular business days and hours, at the Board’s offices at the address stated above. This Notice is given in accordance with the MLUL. Plainsboro Planning Board to Consider Assisted Living Housing

PLAINSBORO TOWNSHIP NOTICE PLEASE TAKE NOTICE THAT on Monday, November 17, 2014, at 7:30 P.M., in the Court Room of the Plainsboro Township Municipal Building located at 641 Plainsboro Road, Plainsboro Township, Middlesex County, New Jersey, the Planning Board of the Township of Plainsboro will hold a public hearing on the application of Hunter Gregory Realty Corporation for Preliminary and Final Major Site Plan with deviation and waiver approval for a 105 unit assisted living and memory care facility proposed to be developed on a 6.1 acre portion of the property currently identified as Block 1701, Lot 4.03 on the Plainsboro Township Tax Map. The 6.1 acre parcel, proposed to be identified as Block 1701, Lot 4.05, is being created under a separate subdivision application. The development site is located on the Princeton Healthcare System Plainsboro Campus just south of Hospital Drive near its intersection with Connector Road a/k/a Campus Road. The property is in the ORC (Office and Residential Complex) Redevelopment District of the Princeton HealthCare System Redevelopment Area. Specifically, the Applicant proposes 105 units in a varied two and three story structure. Fifty-five of the proposed units will be traditional assisted living units consisting of studio and one bedroom units. The remaining 50 units will be primarily studios for early stage dementia and for more advanced memory loss. The Applicant requests the following waivers/deviations from the design standards of the Redevelopment Plan: Section 4.1.5 which requires walkways to be at least 6 feet wide. The Applicant requests approval for some of the internal walks to be 5 feet wide. Section 4.2 to allow minor intrusions of the building and certain architectural elements into the required green corridors of Punia Boulevard and Hospital Drive. Section 5.1.5 which requires glazing of at least 25% of each façade of the building at each individual building level. Section 5.2.1 which requires 105 parking spaces for this project. The Applicant proposes a total of 103 parking spaces, 32 of which will be land-banked for construction if needed in the future. In addition, the Applicant requests a waiver from standards of the Comprehensive Sign approval for the Redevelopment Area for the proposed monument signs. The approved sign detail for the Redevelopment Area consists of internally illuminated brushed metal boxes whereas the Applicant proposes signs using materials and colors drawn from the building exterior with ground mounted lighting. The Applicant requests any additional deviations, variances and/or waivers determined by the Planning Board to be required for the approval of this application. Materials submitted in support of this application are available for public inspection at the Department of Planning & Zoning located in the aforementioned municipal building during normal business hours, Monday through Friday, between 8:30 A.M. and 4:30 P.M. Monroe Township Zoning Board of Adjustment to Hear Changes to Springpoint Senior Living

  MONROE TOWNSHIP
LEGAL NOTICE PLEASE BE ADVISED that Springpoint Senior Living, Inc., as applicant, has made application to the Monroe Township Zoning Board of Adjustment for variance relief and amended preliminary and final major site plan approval for premises known as Lot 17.03, Block 58, which property is located with frontage on Jamesburg-Half Acre Road and is 55.65 plus or minus acres in size and located in the R-20 zoning district of the Township of Monroe. The property is presently developed as an independent living, assisted living and skilled nursing facility consisting of 287 independent living units, 28 assisted living units and 60 skilled nursing beds. The applicant’s proposal is to expand the existing assisted living and skilled nursing facility and to raze a portion of the existing building that currently houses 60 skilled nursing beds. In place of the existing 60 skilled nursing bed facility, the applicant proposes to construct a new 120 bed skilled nursing building and 24 unit assisted living/memory support facility. The applicant’s use of the property as an independent living, assisted living and skilled nursing facility is not expressly permitted by the Monroe Township zoning ordinances in the R-20 zoning district. It is a conditionally permitted use. The applicant does not satisfy all of the conditions. Variance relief for same is required. The applicant shall request variance relief as identified on the plans and in the application including: 1. Setback relief for the proposed skilled nursing building of 25.3 feet where 100 feet is required; 2. Building height of the skilled nursing building of 42 feet (46.5 feet to the chimney) where 35 feet is permitted; 3. Minimum buffer width along right-of-way and property lines of 60 feet where 5 feet is provided along the southerly property line; 4. Parking stall dimensions of 9 feet by 20 feet where ordinance requires 10 feet by 20 feet. The applicant reserves the right to request additional variance and/or waiver relief if same is determined to be necessary or appropriate by the Board or its staff prior to or during the public hearing. Paramus Board of Adjustment to Hear 170 Application for Congregate Care Facility

BOROUGH OF PARAMUS ZONING BOARD OF ADJUSTMENT NOTICE OF PUBLIC HEARING  PLEASE TAKE NOTICE: That an application has been made by Shelter Development, LLC in connection with premises located at 396 Forest Avenue, Paramus, New Jersey 07652, Block 4610, Lots 1, 2, 3.01, 4 and 5 in the R-100 Zone to consolidate the property into a single lot, and to redevelop the property with a new congregate style assisted living, independent living and dementia living facility with a total of 170 units. The existing single family dwellings on the property will be demolished and the entire existing landscaping and nursery business and structures will be removed as well. As such, Applicant requests minor subdivision approval, site plan approval, use and height variance approval, a major soil moving permit and the following variances, waivers and other relief:  1) ?429-22 and ?429-43 list the permitted uses in the R-100 Zone in Paramus, and the proposed 170 unit assisted living, independent living and dementia living facility is not one of the permitted uses in this zone.
